The son of an Islington warehouseman, Timothy Holmes excelled as a student, gaining a scholarship to Cambridge where he graduated BA in 1847. He returned to London and became a student at St George's Hospital. He gained his FRCS in 1853 and served as house surgeon and surgical registrar at St George's Hospital. He also served as curator of the museum and demonstrator of anatomy, and became full surgeon in December 1867. He retired in 1887 but was appointed consulting surgeon and in 1894 he accepted the position of honorary treasurer. He was also elected assistant surgeon to the Hospital for Sick Children in Great Ormond Street in 1859, and was full surgeon there from 1861 to 1868. At the Royal College of Surgeons Holmes served as Hunterian professor of surgery and pathology in 1872, on the court of examiners from 1873 to 1883, and on Council from 1877 to 1885, latterly as Vice-President.
